Federal OSHA recorded a severe workplace injury at Aviation Exteriors Louisiana, LLC, 1218 Hangar Dr, NEW IBERIA, LOUISIANA 70560 on — Fractures , affecting the Thigh(s).

An employee was preparing a commercial jet-liner to be repainted. He had accessed the wing via a scissor lift, and was standing on a flap over an aft wing box, at a height of approximately 15 feet. His harness was on, and he was hooked up to the scissor lift yoyo. He slipped off the top of the wing and fell approximately 6 feet, making contact with the plane and scissor lift, resulting in a fractured left femur.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.

An employee was climbing a ladder to inspect the roof of a church when the ladder slipped and the employee fell. He went to grab the rain gutter along the eave to break his fall when his right hand contacted a sharp edge on the gutter, resulting in a laceration to the right-hand fingers that required hospitalization.

An employee was performing preventive maintenance activities that included cleaning coils on a turbine oil cooler. The employee used a 6-foot step ladder to access the mid-flange area, where he stood to access the oil cooler, apply silicon in-between the flanges, and bolt up the flanges. As he was standing on the side of the cooler, he fell from a height of less than 4 feet but managed to catch himself during the fall. In doing so, his left triceps contacted the threads of an existing stud that secured the top cover, resulting in a puncture wound that required hospitalization.

An employee was descending a fixed ladder into a block house to perform maintenance on a snow-making system. His foot slipped and his heel struck a pipe (part of the snow-making system) behind him. The employee fell partially down the ladder before he stopped himself from falling further and another employee helped him out of the space. The employee suffered a broken right tibia and fibula.

An employee was walking down stairs when he slipped and grabbed onto the railing to prevent himself from falling which resulted in injury to his left hip. The employee was hospitalized.

An employee was on a 12-foot extension ladder while painting the wall and soffit area of a pavilion less than 6 feet off the ground. The employee was using a sprayer when he fell off ladder and landed on his arm on the dirt clay ground. The employee sustained a dislocated elbow and fractures to his right arm and wrist that included an open fracture. The employee was hospitalized and required surgery.

An employee was working from a boom lift, painting wood trim at a second-story roof level. The lift drove over an unmarked dry well, tipped, and catapulted the employee to the ground. The employee sustained fractures to their heels and vertebrae. Fall protection was not being worn at the time of the incident.

On October 2, 2025, an employee was ascending a ladder attached to a scaffold structure approximately 10 feet high. While climbing up, he lost his grip on a rung and fell to the ground, landing on his shoulder. He was hospitalized with fractures to his collarbone and shoulder.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
