An employee was closing the door of a box truck when they fell from the truck to the ground. The employee sustained a punctured lung and fractures to vertebrae and a rib.
An employee finished installing a tube into a 12.4 x 24" tractor tire and connected the air hose to inflate the tube. The tube overinflated and exploded, causing the wheel and tire to fly through the air and strike the employee's forehead. The employee sustained a frontal sinus fracture and a forehead laceration.
