Final narrative

A crew was working at a new warehouse construction jobsite. They were attaching a choker to a hoist. While the rigging was being adjusted, a forklift operator's hand became caught in an unguarded pinch point of the rigging created by the shifting joist, house structure, and/or cable choker. This resulted in the amputation of their left index fingertip.
