An employee was using a pump to pour a concrete foundation at an apartment project. An auger that was supported by a cinder block collapsed and struck them, resulting in a leg and foot amputation and a severe head injury.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.
