An employee was operating a forklift in the receiving area at the rear of the facility. The forklift fell off a portable ramp connected to a receiving truck. As the employee attempted to exit the forklift their leg struck the chain and ramp and they fell two feet to the concrete floor. They were hospitalized for two broken ribs and a bruised left leg.

An employee was feeding fabric into a fabric insert machine. The machine caught her finger and amputated it.

An employee was cleaning the fingers of a spray booth machine. When the employee turned back to the machine, he was picked up by the machine and sustained a back injury.

An employee was moving laser guided vehicles (LGV) due to a jam up. One was in manual when another came up in auto. The employee kicked the sensor to stop the LGV in auto. The LGV reset after 15 seconds and started up again, trapping the employee between the two LGVs. The employee was hospitalized with a laceration and avulsion to the right calf.

An employee was feeding rubber into an extruder and got their hand caught in the conveyor belt, crushing their hand. The belt was not guarded at the time.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

At 2:45 p.m., an employee was outside the plant during a break before his next shift. He was speaking with an associate when he suddenly fell from the third-level steps to the ground, striking his head on the concrete. The employee was hospitalized due to internal brain bleeding, a wound to the back of his head requiring staples, and heat-related dehydration.

Two employees were replacing a motor on an overhead conveyor. The old motor fell to the floor, causing a piece of the concrete floor to fly up and strike the injured employee in the face, around the eye. The injured employee sustained a right orbital fracture, a zygomatic arch fracture, and a sphenoid fracture. The employee was hospitalized and required surgery.

An employee was changing the bladder on a hydraulic press. As he lowered the bladder, his left foot was pinned between its bottom ring and the bottom mold of the press. The second toe on the foot was amputated.

An employee was adjusting a misaligned ring on a tire balancing machine when the ring fell and caught their left thumb against the machine's housing. The employee sustained a fracture and partial amputation of the distal phalanx of the thumb.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
