An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was pulling a metal skid off a stack to place the skid on the floor. The weight of the skid shifted and the edge caught the employee's right middle finger, resulting in a partial amputation.

An employee was operating an order picker in a warehouse. While backing down an aisle, the employee struck a 60-inch-by-60-inch pallet being lifted by another order picker. The employee suffered a tear to the cartilage surrounding their hip.

The driver jumped or slid out of their seat and landed on their feet on uneven concrete ground. Their legs buckled, causing their ankles to twist. The employee was hospitalized with fractures and dislocations to both of their ankles.

The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
