An employee was working with a group of tree trimmers to trim a tree that was approximately 66 feet high and 28 inches in diameter. When the tree fell, it struck the employee. He was hospitalized with multiple fractures to the left side of his body, including his head.

An employee was cutting up a felled tree with a chainsaw. The chainsaw got stuck and the employee turned it off. Another employee stepped between the tree and another tree, pushing down the tree with the chainsaw stuck in it. The tree freed itself from the stump and hit the employee's left knee, fracturing and dislocating it.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

Rae Corporation

An employee was helping to move a 1,300-pound coil on a cart. The steel caster hit a crack in the concrete, the weld holding the caster onto the cart broke, and the coil and cart tipped over onto the employee. He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was rigging I-beams. An I-beam slid off the stack of I-beams and fractured the employee's leg.

On July 18, 2025, at approximately 1:09 PM, an employee was moving a trailer-mounted de-limber. He set his ground saw on the de-limber trailer and then got off the loader and stood to the side of the ground saw. A grapple skidder was raising the trailer slightly so he could throw the chain under the trailer to secure the ground saw. He then turned around and the ground saw slid off the trailer and struck his back. The employee was hospitalized with four fractured vertebrae.

On June 23, 2025, an employee was assisting with a tire replacement. As the employee was returning with tools, two co-workers were positioning the tire on the hub. The tire released pressure, causing a piece of the split rim to strike the employee. They were hospitalized with a fractured left ankle and pelvis.

An employee was cutting a tree. As they were walking away, part of the tree broke off and struck the employee. The employee sustained five broken ribs and a punctured lung.

An employee was climbing down a tree after setting a rope to allow the tree to be removed. A knot at the end of his climbing system failed and he fell from the tree, landing about 35 feet below. He suffered fractures to the pelvis and spinal cord and was hospitalized.

An employee felling a tree using a chainsaw. He was moving away from the tree he had just cut using a planned path, but he was struck by a branch of the falling tree. His collarbone was fractured and it punctured his lung.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.
