Safety Incidents

An employee was walking to a break area when she was struck and run over by an industrial forklift. The employee was hospitalized with an injury to her body and a severe leg injury.

An employee was walking through a dock space on a job site. A car struck the employee with its fender, hood, and windshield. The employee suffered fractures in their arm, wrist, and thumb, as well as injuries to their leg, knee, and head.

An excavator operator and the injured employee were exchanging a hand tool on a utility right of way. After the tool exchange the excavator moved forward and the tracks ran over the big toe of the injured employee's right foot. The toe was crushed, resulting in amputation of the tip.

An employee was walking across the dock, interacting with forklift drivers on what was being loaded. A forklift drove forward with a tall load on the forks. The employee was struck by the load (running board/rocker panel rack) on the forklift. The employee sustained several contusions and an injury to their right Achilles tendon that required surgery.

A crew stopped operations when rainfall started. The injured employee was putting away tools in the tool truck bed while a pickup truck was driving to the end of the location. When the pickup truck applied the brakes, the mud caused the vehicle to slide. The injured employee was caught between the rear bumper of the tool truck and the front bumper of the sliding pickup truck. The employee sustained fractures to their pelvis and femur.
