Final narrative
An employee had been directing plate installs and was finishing up his shift when he became lightheaded. The employee was hospitalized for heat-related illness and dehydration.
An employee was standing on the side of a skid plate while waiting for a welding box to be delivered. A second employee was driving a golf cart to bring the first employee the welding box. The welding box fell onto the accelerator pedal while the driver was approaching the first employee. The golf cart then struck the first employee on the back of his right leg and pushed him onto the skid plate. He sustained a fractured leg.
An employee was walking next to a pipe that was being moved with a forklift when the synthetic strap slipped off the pipe and one side of the pipe fell onto the top of the employee's foot behind the protective steel toe. The employee sustained a dislocated big toe and open and closed fractures.
