An employee was exiting the top of a manufactured home to a catwalk when they fell from the top of the home onto the rail of the catwalk. The employee suffered a broken right humerus and dislocated his right shoulder.

An employee was cutting trim on a miter saw when her finger contacted the blade. She sustained a deep laceration with an open fracture to the right second finger. The finger was surgically amputated at the hospital.

An employee was standing on the top rung of a 6-foot ladder and using a drill to repair a vinyl crown when he reached to one side and fell off the ladder, landing on a house dolly that was adjacent to his work area. He suffered a dislocation and fracture of the left ankle.

An employee was attempting to align the hydraulic mast tilt cylinder on a forklift so he could insert the locking pin. His finger was in the pin hole while he instructed the forklift operator to lower the hydraulic mast so the pin hole would line up and the pin could be inserted. Tactical Demolition LLC

Employees were chipping concrete/brick to separate a building for demolition when the wall collapsed and debris fell onto the employees, resulting in hospitalization of two employees. One employee sustained a fractured left femur and a head injury, and the other sustained a severe head injury.

Form Construction, LLC

On June 11, 2025, employees were completing the installation of flashing and waterproofing material at the base of an existing masonry wall. The wall gave way while the employees were working on it and three employees were injured by falling brick and mortar. Two employees were treated and released. The third employee sustained fractures to both arms, a blunt force injury to the face, and a partial amputation of their right little finger. The third employee was hospitalized.

Two employees were standing under a mobile fall protection gantry span while it was being adjusted. While the security bolts were being removed, the adjustable legs began to spread, causing the main beam to fall and strike the employees on their heads. Both employees sustained blunt force trauma and lacerations to the tops of their heads.

An 18-foot-tall scaffold collapsed onto an employee, who was hit by concrete masonry unit blocks that had been sitting on the scaffold. The employee suffered a broken skull, right hip, and nose, as well as head lacerations and swelling in the right shoulder.

An employee was blowing cellulose insulation into the open attic area of a manufactured home. While the employee was holding the insulation hose, it caught on the guardrail of the catwalk/suspended walkway leading into the attic. The tugging motion caused the employee to lose their footing and step onto the sheetrock ceiling. The employee fell through the ceiling to the ground about 8 feet below and suffered a broken lower left leg. The employee was hospitalized.

An employee was going between a ladder and a wall to repair a wall in the utility room. A brad-nailing gun was hanging on a piece of wood on the side of the ladder. The employee contacted the brad nailer and the trigger was pushed into the wooden piece. The brad-nailing gun actuated and shot a 2-inch brad into the employee's abdomen.

An employee was on a scaffold caulking and painting a manufactured home. She fell off the scaffold an estimated 6 to 8 feet, landing on the concrete floor. The employee suffered upper body injuries and was hospitalized.

An employee was going up a ladder from the first level roof to the second level roof. The ladder slipped and the employee fell approximately 8 to 10 feet to the surface on the first level. The employee sustained fractures to three ribs and bruises on their arms and hands.
