An employee was standing on a piece of equipment called a K-brace, approximately 3.5 feet off the ground. He was working to assemble part of the K-brace. He was driving a metal pin with a sledgehammer and put the sledgehammer on the ground. He then hopped off the K-brace backward and struck the handle of the sledgehammer as he came to the ground. The employee was hospitalized with a laceration to his left buttocks and a fractured tailbone.

An employee fell onto a rig floor while a stand of drill pipe was being moved nearby. The employee had just braced himself, putting his hand out in front of him, when the stand of pipe was set down on the floor. It crushed the employee's left ring finger, resulting in an amputation to the finger.

An employee was standing on the ground while rolling a pony sub from the forks of a forklift onto a catwalk. The sub shifted and fell approximately two feet onto the top of their right safety boot. The employee was hospitalized with a fractured foot that required surgery.

Two employees were carrying a drill bit and installation tool to the center of a drilling rig floor. One of them slipped and fell, and the drill bit and installation tool landed on his left hand. He suffered an open fracture and partial amputation of the last joint of the little finger, as well as a laceration to the ring finger.

An employee was working on a rig floor while connected to a hoist line that was leaning against a derrick held up by a tugger. The tugger fell and a 5-foot sub pipe struck the employee's head, knocking him over. He stood up quickly and his head struck a catwalk trough. The employee was hospitalized with a concussion and a laceration behind their ear with an indented fracture.

An employee was walking through a parking garage. She tripped over a curb and fell forward onto her right knee, then her left knee, both hands, and her face. She suffered an injury to the left knee that required surgery, as well as scrapes on the chin and both palms and soreness in the left wrist.

An employee had been loading packages into a vehicle. While exiting the vehicle, he tripped and fell, fracturing both ankles.

An employee was exiting a train. The employee's foot was caught, and the employee fell, suffering a broken left arm.

An employee was dropping customers off with a shuttle. He stepped out of the shuttle and his foot entered a pothole, causing him to twist his ankle and then fall to the ground. The employee sustained multiple dislocations to the ankle requiring surgery.

An employee was approximately 5 feet up on a ladder while drilling a hole for cable installation. A wasp attacked him. As he was coming down the ladder, he landed flat on his feet on the ground, and his right leg was twisted. The employee sustained a fractured tibia.

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.

An employee was using a torch to loosen a bolt on a water trailer. Natural gas was present in the water and it ignited, causing a fire. The employee sustained third-degree burns to their upper leg.

An employee had been conducting crane operations. The employee went to stop a piece of oil and gas equipment for tubular running from falling over to the ground. Their right hand was caught between a rolling ladder and the equipment, resulting in fractures to the index and middle fingers. They were hospitalized and the middle finger required surgery.

An employee was working to install a drive belt on a water pump. He was holding the belt on both sides of the pulley, working to route it over the top. As he pulled down with his right hand, the pulley rotated and the belt slid over it, pulling his left little finger between the pulley and the belt. He suffered an amputation to the fingertip as well as a fracture to the ring finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
