An employee was delivering beverages when he fell from the truck's liftgate to the ground. The employee sustained a fractured wrist, a dislocated shoulder, and bruised ribs.

At 7:10 a.m. on January 24, 2023, an employee had just cleared a jam on an empty pallet conveyor and was walking back to the palletizer when he noticed that one of the chains conveying the empty pallets was not aligned on its track.
