At approximately 10:00 AM, employees were conducting asphalt loading operations. While securing a hose atop a truck, an employee encountered a malfunction involving a plugged hose. Pressure built up within the hose and suddenly released, causing the hose to fly off the truck. The hose contacted two employees and both employees fell approximately 7 feet. Employee 1 was on top of the truck and sustained three fractured ribs and a head contusion. Employee 1 was hospitalized. Employee 2 was climbing a ladder on the truck and sustained a fractured right leg. Employee 2 was not hospitalized.

An employee was helping to transport plastic material when their left foot was caught in the fabric cutter machine and sustained amputations to their big toe, 2nd toe, and 3rd toe. The employee was hospitalized.

On January 20, 2023, an employee was attempting to clear a jam when his glove was caught and his hand was pulled into the machine, resulting in a partial amputation of the little finger.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

An employee was cleaning the nip roller and setting up/operating a laminating machine when their left hand was caught in the machine and crushed. The employee sustained the amputation of their middle, ring, and little fingers. The machine was not locked out/tagged out at the time.

An employee was working at a coating/printing wind-up station when his right arm was caught in the machine. He suffered multiple fractures to the arm, including a compound fracture.

An employee used a forklift to lift an electric pallet jack so they could diagnose a problem with the pallet jack. The pallet jack fell off the forklift forks and struck the employee. He sustained fractures to the ribs and pelvis. He also fell, landing on a stack of pallets which fractured his left forearm.

An employee was cleaning debris from a machine when their left hand was caught in the in-going nip point. The employee sustained fractures and a crush injury to fingers of the left hand.

An employee was completing a roll converting operation on a machine when their hand/arm became caught in the rollers. Both bones in their forearm were broken.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
