What happened

On [REDACTED] at [REDACTED] an employee came to me and stated that she was reaching to take a magazine off the top of a bucket on the [REDACTED] [REDACTED]. Reaching with her right arm she said she felt pain. She also stated that she has had problems with the right arm hurting in the past and did seek medical attention [REDACTED]. She said the doctor did an X-Ray but did not let her know what the problem was and she is still working. She did go seek medical treatment due to the incident that occurred today [REDACTED].
