What happened

Rear left tire of [REDACTED] fell off vehicle while employee was travelling to service [REDACTED] [REDACTED]. He was travelling on [REDACTED] coming up to the intersection with [REDACTED] when the tire fell off the vehicle. The vehicle came to an abrupt stop and swerved slightly to the right side of the road. This resulted in the employee hitting his right knee under the dash board and causing pain and discomfort in his right knee. Physician later confirmed that the employee suffered patellar tendonitis to his right knee.
