On [REDACTED] at approximately [REDACTED] an employee slipped and fell onto the floor due to them having been freshly waxed. While he was on the floor a RCA who was on [REDACTED] asked if he was okay he replied no that hurt. As the employee got up. he had the black wax all over his right leg his left knee hands and buttocks.
