What happened

On [REDACTED] @ [REDACTED] EE was changing a coil over at the intercept machine in the glass department. As the EE was going through the steps of the change over. EE Checked for proper placement of the cartridge before proceeding and everything looked good. As she went to place the cover on the cartridge became loose and fell to the ground striking her left toe in the process.
