What happened
EMPLOYEE NOTICED A SMALL LEAK COMING FROM A TOTE THAT WAS STACKED ON TOP OF ANOTHER TOTE. HE THOUGHT IT WAS THE VALVE HE USED A TOOL (TOTE WRENCH) TO HELP CLOSE THE VALVE THE TOOL SLIPPED OFF THE VALVE CAUSING IT TO STRIKE HIM ON THE LEFT EYEBROW.
