What happened

Walked a few steps and right foot landed in a small pile of shortening that had fallen on the ground [REDACTED] before. Fell head first to the ground. Forehead hit the electric pallet jack 's outrigger which caused a contusion to forehead.

Injury or illness

Two contusions on forehead caused bleeding. Stitches applied at ER.

Shortening pile fell on the ground a minute before the injury occurred. Landed in shortening pile with right foot and slipped forward to ground hitting outrigger.
