Before the incident

EE was attempting to clean a dirty printing plate on Bag Machine #626. EE was maneuvering the Plate Cylinder into position to clean the printing plate when the rag wrapped around their right hand was pulled into the [REDACTED] Roller. EEs right pinky and ring fingers became lodged between the Printing Plate and [REDACTED] Cylinders.

What happened

EE was attempting to clean a dirty printing plate on Bag Machine #626. EE was maneuvering the Plate Cylinder into position to clean the printing plate when the rag wrapped around their right hand was pulled into the [REDACTED] Roller. EEs right pinky and ring fingers became lodged between the Printing Plate and [REDACTED] Cylinders causing a right pinky fingertip partial amputation and right ring fingertip degloving.
