Before the incident

The employee was told by the supervisor ([REDACTED]) that FM02 was presenting issues with the plastic falling off the chain rails. The employee made his way to FM02 infeed and was trying to identify the issue at hand.

What happened

As soon as the employee placed his left hand on the plastic feeding through the infeed the machine advanced forward pulling in the employee 's left hand through the rollers. That caused a fracture to his bone.
