Bending over to hook a plate up to overhead for lifting

What happened

Team member was working under the log haul chain preparing to remove a plate. He bent over to hook the plate up to the overhead and when he did he reported that he felt his back lock up. When he stood straight he felt radiating pain down his back and legs. His coworker assisted him back to the shop where he reported the incident to his supervisor. Team member was asked if he wanted to seek medical care and he stated no that he wanted to see if it worked itself out over the weekend and we would reassess the situation on [REDACTED]. Team member was sent home to rest and was advised by safety to call if anything changed over the weekend and or he changed his mind about getting medical care. A packet was completed and a drug test administered before employee left. Employee was sent home at approximately [REDACTED]. On [REDACTED] employee called in to supervisor stating that he wasn 't able to get out of bed but still refused to seek medical care. This was mis-managed and not reported throu
