What happened

An employee placed a handheld grinder on a work table with the blade facing upward. While reaching across the table the employee’s arm inadvertently came into contact with both the grinder and its trigger. The grinder’s trigger safety mechanism had been either modified or damaged disabling the safety lock feature.
