What happened

The arm of the [REDACTED] clamp used to hold one side of the flange into position extended across the opening of the steel pipe where the employee was swinging the hammer. Employee 's hand holding the hammer handle struck the edge of the [REDACTED] clamp during a swing pinching the tip of his finger.
