Before the incident

The employee had wrapped the poly roll with tape by turning on the air to spin the roll attaching tape then walking it down to the end of the roll. This makes the tape wrap around the roll so it spins.

What happened

When the employee got to the end of the poly roll they reached the tape up grabbed the knife and cut the tape. The knife ended going upwards too and sliced open the top of the hand through the glove
