What happened

[REDACTED] LCSW was in session with her 2pm patient on [REDACTED] when she tripped and fell to her knees then forward hitting her face on a wood play table. The patient and his mother asked if she needed help and at that time [REDACTED] declined and continued the session. It was [REDACTED] during the session when both patient and mother noticed [REDACTED] starting to slur her speech and start nodding in and out. The patient rushed to the front desk and told them they needed help. [REDACTED] went to the office and pressed the duress button. [REDACTED] Clinic Manager and [REDACTED] MA arrived [REDACTED] later. [REDACTED] RN was contacted and he arrived at [REDACTED] and conducted a neuro assessment and advised us to call [REDACTED] which [REDACTED] did. Security [REDACTED] and [REDACTED] also arrived for support. EMT arrived at [REDACTED].
