Operating a forklift to stage a pallet in a warehouse bay

What happened

After staging a finished goods pallet in a warehouse storage bay the employee began to back out of the aisle and did not see the I-Beam prior to striking with the lift. When the operator hit the beam he was thrown from the lift and struck is forehead on the concrete after hardhat fell off.
