Millard Fillmore Suburban Hospital on the date of incident (2 6 25) diagnosed closed head injury and forehead abrasion. HealthWorks Clinic subsequently diagnosed (2 10 25) concussion without loss of consciousness and strain of muscle fascia and tendon at neck level with restrictions which could not be accommodated.
