What happened

Associate was performing a die change at [REDACTED] in APDC. Associate was standing on the fixed side of the die and moved to hook up the moving side. Associate slipped on the deck plate and his leg went in between the plate and the tie bar where he received a small puncture on his right shin from the corner point of the deck plate.
