What happened

TM was lifting an Anchor ( 0057216) 16 KG S S DTX ANCHOR 35 LB out of a crate. The Anchor got stuck. He then proceeded to lift the Anchor up and when he did the motion of lifting the Anchor out of the crate was enough to have the Anchor come up and hit him in the mouth which caused him to chip his tooth and a small cut inside his mouth with a swollen lip.
