DART- On [REDACTED] [REDACTED] reported having some back pain from running over potholes outside. [REDACTED] felt fine the days following with some discomfort at home but was able to preform their job. At [REDACTED] on [REDACTED] [REDACTED] (forklift driver) was driving hilo #3 at the outside west concrete storage pad [REDACTED] has to frequently drive outside to dump scrap hoppers and retrieve empty pallets. [REDACTED] hit a pothole outside with empty pallets on the forks of #3 hilo upon hitting the pothole [REDACTED] shifted forward and bounced up in the forklift seat. [REDACTED] then reported to their supervisor that their lower left side of their back was sore [REDACTED] initially felt neck pain but it subsided. [REDACTED] requested first aid and bio-freeze gel was applied. EHS checked in with [REDACTED] around [REDACTED] [REDACTED] and [REDACTED] felt better. Supervisor checked in with [REDACTED] around [REDACTED] after lunch and they felt fine still and did not assistance with jo
