Moving patient to ED CT scanner.

We brought in pt code stroke pt to ED CT scanner; I was getting slide to assist in moving pt over to bed; CT tech noted pt had hover mat; I put slide board up and was returning to assist with moving pt to scanner table when my right foot caught on rug ct flat insert trash can and I fell forward landing on both knees L hip and R elbow.
