Before the incident

Cutting a strip of steel out of a stamping die to scrap it out for a changeover to another job

What happened

As the employee was shearing the steel strip into smaller sections to scrap it out one of the sections that was sheared off was cut through a small cutout area in the strip creating a small spiked area that the employee didn 't see when he was handling the smaller sections to put into a scrap chute and the spiked are punctured the palm of his left hand cutting it.
