What happened

The employee was conducting his routine QA fiber checks when he moved next to the accordion conveyor transfer belt to place cases back into the conveyor line. When the employee stepped back to take the fiber sample to the QA office he was unaware that a pallet was behind him causing both of his heels to get caught in the pallet causing him to trip and fall resulting in pain injury on his lower back.
