Before the incident

The employee had finished removing their gowns inside the Gowning Room. They stood up from the bench and began to walk toward their cubby to store their garments.

What happened

The employee was preparing to go to their first break of the day. They entered the Gowning Room from the Cleanroom side and finished ungowning. Upon doing so they began walking to their cubby to store their garments. The tip of their right foot came into contact with the floor causing them to fall forward.

Injury or illness

The employee sustained bilateral knee injuries as well as pain in their right arm right shoulder and right elbow.

The vinyl-covered floor.
