Before the incident

An employee entered the AHI area of the winder to cut the tape holding a 6 side roll to the parent roll and transfer the side roll to the down ender The side roll started to fall in the direction of the person and they tried to move out of the way When the person tried to back up their left ankle hit a wedge in the floor and the employee fell backwards The roll landed on the persons ankle The approximate weight of the side roll was 650 lbs
