The employee was cleaning the machine.

What happened

While cleaning machine #539 an employee was injured due to unintended access to a moving part of the machine. The safety switch inside the machine was broken and the temporary countermeasure—fastening the access panel door closed—was compromised when the fastener was removed. During cleaning the air hose contacted a moving component startling the employee who then pulled back and struck their hand on nearby equipment.
