Before the incident

Training a teammate on the wheel hub assembly machine. A part the trainee loaded was off center and the teammate assisted the trainee.

What happened

Teammate reached through a guarding gap to center the par then asked the trainee to activate the two-finger touch. Teammates index fingertip was still in the gap between the assembly piston and wheel.

Fracture of distal phalanx of right index finger. Crushing injury of right index finger. Laceration of right index finger.
