getting phone out of her car

What happened

Housekeeping employee was walking back from her vehicle to retrieve her phone when she slipped and fell on the sidewalk. apparently the sprinkler water had frozen over on the sidewalk due to the cold weather. Employee called me at [REDACTED] to tell me that she had fallen and couldn 't get up. her two leaders immediately went over to help employee she was laying on the sidewalk off to the left of the Admissions entrance she did complain of pain in her back. She was transported to Eisenhower medical center by paramedics. Nursing also responded and called [REDACTED]. there were no witnesses.
