What happened

EE states he arrived at the shipper and observed their parking lot was covered with snow ice. EE stepped out of his truck to disconnect his airlines and slipped on the ice. EE fell to the ground landing on his right elbow and states this jarred his right shoulder. EE did not report it as he thought the pain would go away but it did not. EE sought initial treatment with his PCP on [REDACTED] then again [REDACTED] at North Louisiana Orthopaedic Sports Medicine Clinic [REDACTED]. EE has been taken off work pending an MRI.
