Before the incident

EE was installing a side-lite spaces on an Envisions door frame which requires a hammer to put it into place. EE was using a bead hammer instead of a dead blow hammer making the task more difficult than it should be. The EE swung the hammer to put the spacer in place missing the spacer with the hammer swinging up striking her right front tooth resulting in a chip to the corner if the tooth. The EE went to a health clinic who released her to duty and instructed her to follow up with a dentist. EE did report the incident immediately but did not clarify that it occurred at work. Her lead thought it occurred outside of work.
