What happened

Employee reports was transferring patient with a gait belt with wife from his wheelchair to his bed. The patient had a tight hold on the wheelchair and while trying to transfer he was very weak. The patients hospital bed doesn 't quite work correctly; and the employee had to lift the patient a little higher than normal hurting her right shoulder and right upper and lower back.
