Before the incident

I was sitting in the chair and proceeded to get up to assist someone at the exit side door of the [REDACTED].

What happened

I was trying to get up out of the chair and the chair slid from under me and flipped over (NOTE- there is a small piece of carpet on the floor that often slides around)). The chair landed on top of me. I hurt my right shoulder trying to catch myself and landed on my right hip. (The portal would not allow to put both areas that are injured). I was informed by the Administrative Assistant at the Commons front desk to be sure to complete the report-[REDACTED]).
