Before the incident
The employee had loaded a gondola cart into the mechanism that is designed to lift and dump the gondola cart contents.
What happened
The employee was putting a VMAG gondola cart of meat into the mixer. After the VMAG started to lift the employee noticed that it began to slide. The bar guard on the mixing elevator hit the employee in the mouth.
