What happened

Lifting a box on top of the pallet I felt a pop in the back right after and it got worse over the next [REDACTED]. No equipment is involved in the box on the pallet. The box was too heavy to lift. Lead reported to HR and [REDACTED] was sent to urgent care at Liberty [REDACTED]. I received paperwork that is also attached to this email. Incident report also attached.
