What happened

While moving a box weighing approximately 50-60 lbs the employee experienced pain in their lower back. The employee did not report this at the time of the incident and it went unreported until [REDACTED]. On [REDACTED] the employee had an unrelated appointment with their primary care provider who prescribed muscle relaxers for the back pain. The employee reported this to their supervisor who then reported to HSE. Further evaluation by an occupational health provider determined that further actions were unwarranted.
