What happened

Employee was working on patching the PT pockets with the assistance of a scissor lift. As he attempted to move the lift while standing on the floor next to the scissor lift with the control box in hand and did not notice that the wheels were turned sideways and when he hit the forward backwards button the lift moved quickly and the wheel struck the back side of his left foot crushing it.
