Before the incident

The employee had climbed a 8ft portable A-frame ladder to screw the inner liner on a building.

What happened

Upon climbing the ladder the employee noticed that ladder was unstable so he moved it to stabilize it. The employee climbed the ladder a second time and ascended to the third step down from the top. While screwing in the liner the ladder tipped over after its foot rolled into a 4-way crack in the concrete floor.
