Per Investigator description - EE was picking Ford Business in [REDACTED] aisle when a wheel to the break truck fell off causing him to come to a sudden stop. When doing so EE fell forward hitting his stomach on the handles of the PMHV. [REDACTED] Registered Nurse [REDACTED] [REDACTED] Received EE s statement - I was driving a crown brand tug machine where everything is fine with my load. On the [REDACTED] line the cart I had lost a wheel. It came out of its place and the machine I had stopped suddenly and hit me in the part of the stomach on the left side.
