the bagger completed end seal on film splice which resulted in not sealing the bag. employee did another end seal cycle after completing 2nd cycle she went to front of sealing chamber waiting for bag to discharge. once the bag was moved it tripped the limit switch causing the stacker arm to lower and hit EE on head
